For Level D-Field Trip Chaperone Level D Application
Forms to complete and return:
- Volunteer Application
- Volunteer Code of Conduct
- Current Negative TB Test (must have been completed within the last four years)
- If you were a level B or D volunteer in 24/25 and your TB is still active, we are able to verify it for you.
- Valid Government Issued Picture ID or Driver’s License
o First time Level D Level D also requires Livescan. Livescan must be done on our district form, Livescan for other agencies (other school district, FBI, DOJ, etc.) are not visible by San Diego Unified School District. Applicants must go to a Livescan location, you can google a location that is convenient for you, take the form we provide, and return the form so we can process it as part of the application the front office submits to the District for approval. The cost is approximately $80 at most Livescan locations and the district does not reimburse for these fees.
o Returning Level D requires all of the above, with the exception of the Livescan if it was completed in the 2024/2025 school year. If your Livescan was on file and you were approved as a Level D volunteer last year, then you will mark the application as Returning Level D and we will indicate that your fingerprints are on file.
Additional Information:
- Once we received your documents you will be vetted through Megan’s Law.
- All documents must be provided to the McKinley front office; no partial applications will be accepted.
- Processing times are 4-6 weeks.
